I hereby grant 5th Photography, LLC permission to photograph me in the attire provided at the photoshoot. I hereby assign full copyright of these photographs to the 5th Photography (and the related representatives) together with the right of reproduction either wholly or in part. I grant to the 5th Photography or licensees or assignees the permission to use the above-mentioned photographs either separately or together, either wholly or in part, and grant the perpetual and irrevocable and unrestricted right to use and publish video and/or photographs of me, or where I may be included for editorial trade, product advertising and such other fashion/business purpose in any manner and medium. The Photographer and licensees or assignees may have unrestricted use of these for whatever purpose, including advertising, with any retouching or alteration without restriction. I agree that the Photographer or any person authorized by or acting on his or her behalf may use the above mentioned photographs or any reproductions of them for any advertising purposes or for the purpose of illustrating any wording. I undertake not to Prosecute or to institute proceedings, claims or demands against either the Photographer or his or her agents in respect of any usage of the above mentioned photographs. I hereby release the photographer named above from all claims and liability relating to images, video or photographs taken of me. I have read this model release form carefully and fully understand its meanings and implications.

You, the Model, have received this model release because you have been photographed or have agree to be photographed by 5th Photography, LLC, DBA Steph Kadlicko, and some or all of your images will be used for promotion via website, blog, social media, and/or print by 5th Photography, or any DBA entity under or with 5th Photography, including Steph Kadlicko.

By signing this release, you, the Model, grant permission for these images to be shared and used publicly. You, the Model, also individually have permission to use the images for personal purposes in electronic web-size format images only for such activities as personal branding and social media. ALL IMAGES MUST BE CREDITED TO THE PHOTOGRAPHER - WITH NO EXCEPTIONS (including all social media, for all purposes including birthdays, daily posts, blog posts, etc.). Images (full size digital and print) and all products (including, but not limited to books, albums, magazines, other print media, enlargements, and photo boxes of any kind) MUST be ordered through 5th Photography/Steph Kadlicko, exclusively. No print reproductions from any other source are permitted without explicit written permission from 5th Photography/Steph Kadlicko. Images are not permitted to be sold or exchanged for money of any denomination, at any time, and may not be published in print publication without explicit written contract from 5th Photography/Steph Kadlicko.
